Tier 4

By proceeding with this purchase, you acknowledge the following:

The characters that you have submitted are as you wish them to be printed on the tile(s).
All words in Tiers 1-3 will be printed in uppercase letters only.
This purchase is final, and there will be no refunds in case of future rezoning, moving, or any other reason.
If you want any Tier 4 names to be on the same tile, you must order them at the same time, on the same order form. No other accommodations can be made to place student names with any other student names, unless they were ordered at the same time, on the same order form.
You cannot select where the purchased tile(s) will be placed on the legacy wall.

To pay by check, please complete the order online, but instead of completing credit card payment, write a check to "Creekside PTA" and send to the Creekside Elementary office (list "Legacy Wall" in the memo line).
